Getting Paper Copies

At any time, you may request from us a paper copy of any record provided or made available electronically to you by Elite 360 Innova Healthcare.
You will have the ability to download and print documents we send to you through the Wix system during and immediately after the signing session. If you create a Wix client account, you may access these documents for a limited period (usually 30 days) after they are first sent to you.

After this time, if you wish for us to send you paper copies of any such documents from our office, you may request them using the process below. A fee of $0.00 per page will be applied for printing and delivery.

Withdrawing Your Consent

If you initially consent to receive notices and disclosures from us electronically, you may later change your mind and choose to receive all future notices and disclosures in paper format instead.

You can inform us of your decision to withdraw your consent by following the steps outlined below.

Consequences of Changing Your Mind

If you elect to receive required notices and disclosures only in paper format, this may slow the speed at which we can complete certain processes or provide services to you.
To indicate that you are changing your mind, you must withdraw your consent by using the  “Withdraw Consent” form on the signing page, rather than signing it. This will notify us that you have withdrawn consent to receive documents electronically, and you will no longer be able to use the Wix system to sign or receive forms digitally from us.

All Notices and Disclosures Will Be Sent Electronically

Unless you notify us otherwise, all required notices, disclosures, authorizations, and other documents will be sent electronically through the Wix system.
We recommend this method to reduce the chance of missed communications. If you prefer to receive these notices and documents by mail, please contact us using the instructions below.

In addition, please update your email address directly within your Wix account settings to ensure your information remains accurate across our system.

To request delivery of paper copies of any previously provided electronic documents, please send an email to info@elite360beauty.com and include:

• Your email address

• Your mailing address

• Your phone number

 

We will process your request promptly and notify you if any fees apply.

To Withdraw Your Consent

To withdraw your consent for electronic documents, you may:

1. Decline to sign a document from within your Wix session and select the checkbox indicating you wish to withdraw consent, or

2. Send us an email at info@elite360beauty.com stating your full name, email address, mailing address, and phone number.

 

Please note that withdrawing consent for electronic records may cause some delays in service processing.
